Medical

All New Students need to complete the Academy Student Medical Form that is in the FACTS Family Portal under Web Forms. The form has been updated to include authorization for Over-the-Counter Medications. Please make sure you type your signature at bottom of the form and click save before exiting.

Returning students only need to complete the Academy Student Medical Form if it was not previously completed or if updates are required due to changes in medications, allergies, conditions/diagnoses, or over-the-counter medication authorizations. The form is in the FACTS Family Portal under Web Forms. Please make sure you type your signature at bottom of the form and click save before exiting.
