To access this form, visit the FACTS Family Portal under “Web Forms.” The form has been updated to include authorization for Over-the-Counter Medications. Please make sure you type your signature at bottom of the form and click save before exiting. Returning students should only complete this form if it was not previously completed or if updates are required due to changes in medications, allergies, conditions/diagnoses, or over-the-counter medication authorizations.

If applicable, fill out the Authorization to Self-Administer Form. Please complete this form if your child needs permission to carry and self-administer EpiPens or inhalers. (Students are not allowed to carry any other medications.) If your child will be carrying their own EpiPen, please provide the second EpiPen to the clinic.
