→  Academy Student Medical Form
To access this form, visit the FACTS Family Portal under “Web Forms.”
New student families must complete this form in full, including insurance, allergies, conditions/diagnoses, prescription medications, and over-the-counter medication authorizations. Type your signature at the bottom of the form and click Save before exiting.
Returning student families only need to complete this form if it was not previously submitted or if updates are required. If making updates, please type your signature and click Save before exiting.

→  If applicable, fill out the Medication Verification Form.
Complete this form if you are providing prescription medications (including EpiPens) to Academy at the Lakes. Please print, complete, and return this PDF form to Nurse Getta along with the medication.

→  If applicable, fill out the Authorization to Self-Administer Form.
Please complete this form for Middle and Upper Division students who need permission to carry and self-administer an EpiPen or inhaler. Students are not permitted to carry any other medications. If your child will be carrying their own EpiPen, please provide a second EpiPen to the clinic. This PDF form must be printed, completed, and returned to Nurse Getta.
