Medical

All New and Returning Students need to complete the Academy Student Medical Form that is in the FACTS Family Portal under Web Forms. The form has been updated to include authorization for Over-the-Counter Medications. The form needs to be filled out even if you have previously filled out a Medical Form for your student in FACTS. Please make sure you type your signature at bottom of the form and click save before exiting.
